They are so large in a lead II rhythm strip because this woman with COPD has R atrial enlargement and these are resulting \u201cP pulmonale\u201d. This rhythm is multifocal atrial tachycardia (MAT).\n<p><a href=\"http:\/\/torreyekg.com\/wp-content\/uploads\/2015\/11\/Screen-Shot-2015-11-01-at-6.14.00-PM.png\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-322 size-full\" src=\"http:\/\/torreyekg.com\/wp-content\/uploads\/2015\/11\/Screen-Shot-2015-11-01-at-6.14.00-PM.png\" alt=\"Screen Shot 2015-11-01 at 6.14.00 PM\" width=\"359\" height=\"107\" srcset=\"https:\/\/torreyekg.com\/wp-content\/uploads\/2015\/11\/Screen-Shot-2015-11-01-at-6.14.00-PM.png 359w, https:\/\/torreyekg.com\/wp-content\/uploads\/2015\/11\/Screen-Shot-2015-11-01-at-6.14.00-PM-300x89.png 300w\" sizes=\"auto, (max-width: 359px) 100vw, 359px\" \/><\/a><br \/>\nMAT \u2013 rapid, irregularly irregular rhythm with P waves of varying morphology<\/p>\n<p>MAT can be caused by several conditions including: electrolyte abnormalities, exacerbation of respiratory disease, drug toxicity (theophylline toxicity), and almost any severe illness \u2013 it is said that MAT is one of the common arrhythmias noted in an ICU population.<\/p>\n<p>This patient had recently been discharged home after an admission for exacerbation of COPD, but had no respiratory complaints on this presentation.
